Registering with the Surgeries
We currently have an open list and welcome requests for registration with the practice from patients living or moving to the practice area (please click to view).
Please speak to our Reception Staff who will ask you to complete two forms.
GMS1 form (Department of Health): it requires your previous address and name of previous GP.
Practice Registration Form: on this form you have to fill in your previous medical history. It will be the only information we will have about you until your notes arrive from your previous GP.
Please bring in a form of identification, eg. passport or identity card, to register!
All new patients joining the Heathfield Surgery will be invited to attend an appointment with a Practice Nurse to undertake a health check. This enables us to get some basic details about your current health status. If you are on repeat medication, you will also be asked to make an appointment with a GP to do a brief medication check and to enter your repeat prescription drugs onto the computer.
Temporary Residents
We welcome patients to join our list as a Temporary Resident if they are residing outside their own GP’s area for more than 15 days but less than 3 months. You will be required to sign a Temporary Resident Form and give some details of your usual GP and your intended length of stay. When the Temporary Resident period is over, a copy of your clinical notes will be forwarded to your own GP so that their records are complete.
Removing Patients from our list:
There are 2 reasons why patients may be removed from our list:
Out of Area - Should you move to an address that is outside our practice area we will write to you to inform you that you may no longer be registered at the practice as you live outside of our area. We will provide you with information to help you find another G.P.
Violence and Aggression - The Partners at the Heathfield Surgery re-affirm their commitment to do everything possible to protect staff, patients and visitors from unacceptable behaviour and have a no tolerance of any incident that causes hurt, alarm, damage or distress. Appropriate action will be taken by the practice which may involve removal from the practice list and/or involvement of the police.
