Registering with the Surgeries
|
We currently have an open list and welcome requests for registration with the practice from patients living or moving to the practice area (please click to view). Please speak to our Reception Staff who will ask you to complete two forms.
Please bring in a form of identification, eg. a passport or identity card, to register! All new patients joining the Heathfield Surgery will be invited to attend an appointment with a Practice Nurse to undertake a health check. This enables us to get some basic details about your current health status. If you are on repeat medication, you will also be asked to make an appointment with a GP to do a brief medication check and to enter your repeat prescription drugs onto the computer.

| Temporary Residents |
We welcome patients to join our list as a Temporary Resident if they are residing outside their own GP’s area for more than 15 days but less than 3 months. You will be required to sign a Temporary Resident Form and give some details of your usual GP and your intended length of stay. When the Temporary Resident period is over, a copy of your clinical notes will be forwarded to your own GP so that their records are complete.
